Use this form to submit changes in your name or contact information.

Membership Status Change Request
Use this form to submit changes in your membership status (such as pending retirement, leaving Unit4, "temporary separation" from the CSU such as a leave of absence, and returning to Unit4 after a permanent or temporary separation). Since your participation in the APC/LIUNA Pension Plan could be affected by a change in membership status, this form also enables you to notify us of your wishes regarding the Plan, as well as to request Plan information based on your situation.

A brief note on confidentiality and security: Our on-line forms are confidential and secure. Data you submit electronically will be accessible only by APC. However, if you prefer, you may complete a form online, print it, and then fax the hard copy to APC's Statewide Office at 209-632-3264. (If your signature is required, you will need to print, sign and mail the form.)
