Use this
form to notify APC of a change in your own membership
status
due to a change in your Unit 4 status.
To
notify APC of a change in your name or contact information,
please use the Member Information Change
Request.

Note: If you have never been a member of Academic Professionals Employee or an employee in Unit 4 at the Calfornia State University, do not use this form. Instead, please contact your own union directly. Thank you.
THIS IS NOTIFICATION THAT I AM OR
WILL BE: (select

Retiring
from (leaving) the CSU.
Leaving
Unit 4 but continuing as a CSU employee.
On
temporary leave but not leaving Unit 4.
Returning
to Unit 4.
Note: This form cannot be used to cancel your membership while you
remain in Unit 4. It's important to understand thatby
lawa fair share fee deduction must
be taken whenever a paycheck is issued to a non-member for a Unit 4 position.
Therefore, since the membership dues and fair share fee deductions are both
currently 1.0% of gross salary, resigning
your membership will not result in any savings unless and until
you qualify for the reduced fee
during the next fair share cycle.
