This area of our site is for dues-paying APC members—and for those seriously considering becoming members. Any member of APC (LIUNA Local 1002) can use the forms below. (If you're not a member yet, use the first form to join now!)

APC Membership Application
Are you a "member in spirit" who's ready to become a member in fact? Ready to stop paying a 1% fair share fee when you could pay the same 1% for membership dues instead—and enjoy the privileges and benefits of membership at no extra cost? Just complete and mail this application—and join your Union today! (If you haven't installed Acrobat Reader, or if you haven't upgraded to at least Acrobat Reader 5, we recommend that you do so now, before going to the application. Member Information Change Request
Use this form to submit changes in your name or contact information.

Membership Status Change Request
Use this form to submit changes in your membership status (such as pending retirement, leaving Unit 4, "temporary separation" from the CSU such as a leave of absence, and returning to Unit 4 after a permanent or temporary separation). Since your participation in the APC/LIUNA Pension Plan could be affected by a change in membership status, this form also enables you to notify us of your wishes regarding the Plan, as well as to request Plan information based on your situation.

LIUNA Women's Caucus Membership Application
Any APC member—regardless of gender—can join the LIUNA Women's Caucus. And there are no membership fees or dues! For more information on this recently formed group, see our FAQs on the Women's Caucus.

A brief note on confidentiality and security: Our on-line forms are confidential and secure. Data you submit electronically will be accessible only by APC. However, if you prefer, you may complete a form online, print it, and then fax the hard copy to APC's Statewide Office at 510-482-1160. (If your signature is required, you will need to print, sign and mail the form.)

This area of the site is under construction. We plan to build on it, making it easier for members to keep their information current with the Statewide Office and to get information they need about the benefits of membership.
